Registration Open Now

Registration is required. Fit testing is $40 for small farms and $78 with annual gross receipts greater than $350,000. Email FitTest@bassett.org or give us a call at 800-343-7527.

To Register, you will need the following information:

• Full name (first/last) of person being fit tested
• Make, model, and size of respirator being fit tested
• Respirator type (full face, half-face elastomeric, disposable face filter)
• Documentation of completed medical clearance
  • For a convenient online option for medical clearance, visit 3M’s medical evaluation tool

If you opt to have your medical clearance completed by your Prime Care provider, please contact us so we can send you the necessary documents to take to your appointment.

Registration Pro Tips!

1. First come first serve for appointments. Appointments are not scheduled until complete names of who will be tested and their respirator details are provided. Incomplete information can result in split appointment times for your crew!
2. Medical Clearance.
   1. When choosing 3M online medical clearance: during registration chose two or three year clearance! It is the same price and will save steps next year!
   2. When using your preferred provider: ensure the provider has filled in the renewal year on the Statement of Medical Clearance. If they miss this, the renewal will default to one year! Save steps next year and let your provider choose when medical clearance needs to be evaluated again.
